If your child has seizures or a diagnosed seizure condition, please download the "Seizure Information" forms  and complete them with your child's physician. Return the forms to your child's school, Attention: District Nurse.
 

A Health Care Plan will be created for your child; the District Nurse will incorporate information from the forms you return to the school and will contact you to discuss the plan, ask for your review of the plan and explain further the management of seizures in Los Altos School District.
